What the Assessment Includes: Personalised Assessment:
A qualified Occupational Therapist will complete a comprehensive OT assessment.

Recommendations for Equipment:
Suggestions for equipment to support with bathing, toileting, bed transfers, stairs, seating and access to your property.
Home Adaptation Guidance:
Tailored advice on modifications to improve accessibility and safety, such as ramps, stairlifts, or bathroom adjustments.
DFG Report Preparation:
We provide detailed OT reports to support your application for a Disabled Facilities Grant (DFG), helping secure funding for essential home improvements.
Provision of FREE rehousing report forms if your property cannot be adapted.​
